If your current driving licence or learner permit is lost, stolen, damagedor defaced, you should apply to the National Driver Licence Service (NDLS) fora replacement. The replacement licence or permit is a copy of your originaldriving licence or learner permit.
If there are any special drivinglicence codes on your original licence or permit (for example that you mustwear glasses when driving), the replacement will also show these codes. Thereplacement licence or permit will be valid for the same period as youroriginal.

Rules
You must carry your driving licence or learner permit with you at all timeswhen driving in Ireland. Failure to produce your licence or permit when askedto by a member of the Gardaíis an offence.
If your driving licence or learner permit has been lost, stolen, damaged ordefaced and it is still in date, you can apply for a replacement by completingapplication form D401 for a driving licence or D201 for a learner permit. See‘How to apply’ below for more details. The form must be signed and stampedby a Garda in your local Garda station.
A driving licence or learner permit will be issued for the time remaining onthe original licence or permit.
Expired licence or permit
If your most recent driving licence or learner permit has been lost, stolenor damaged and is out of date, you can only apply for a new licence or permit.You can apply online or complete application form D401 for a driving licence orapplication form D201 for a learner permit. See ‘How to apply’ below formore details. The form must be signed and stamped by a Garda in your localGarda station.
Rates
The fee for a replacement driving licence or learner permit is €35.
The fee for a new driving licence or learner permit if your licence orpermit has been lost, stolen or damaged and is out of date is:
- Ten-year driving licence - €55
- Five-year bus or truck driving licence - €55
- Three-year driving licence - €35
- One-year driving licence - Free
- Learner permit - €35
Motorists over the age of 70 can get their driving licence or learner permitfor free.
If you are paying online you can pay by credit card, debit card, Google Payor Apple Pay.
Any required eyesight tests or medical examinations are notfree of charge.
How to apply
You can apply online or in person at an NDLS office.
Applying online
You will need the following to apply online:
- A Public Services Card and verified MyGovID
- A completed Garda Declaration Form
- Proof of your address if your address is different to the one you provided when you got your Public Services Card
- Proof that you are normally resident in Ireland if you are not an EU or EEA citizen
- A medical report form if required
- Application fee of €35 (unless you are 70 or over)

Applying in person
During Level 5. you should only book an appointment to attend an NDLS centreif you need your licence because you are an essential worker.
Application forms are available from NDLS centres, driving test centres,driving theory test centres and Garda stations. You can also download them:

Section 15 of the application form must be signed and stamped by a Garda inyour local Gardastation.
You must bring your completed application form along with the followingdocumentation:
- Your current or most recently issued driving licence or learner permit, if your licence has been damaged rather than lost or stolen
- A Driving Licence Medical Report Form may be required (see below)
- A Driving Licence Eyesight Report Form may be required (see below)
- The appropriate fee - see ‘Rates’ above
- Documentation to prove your identity
If you have a Public Services Card, you can use this to prove youridentity.
Medical report form
You will need a Driving Licence Medical Report Form if:
- You are applying for driving licence categories C, C1, D, D1, CE, C1E, DE or D1E (unless you previously provided a medical report which is still applicable)
- You will be 70 years of age or more on the first day of the new licence period
- You have any of certain disabilities or conditions (see application form)
You can downloadthe medical report form (pdf) (it must be printed back to back on onepage). It is also available from NDLS centres. A registered medicalpractitioner should carry out your medical examination and then complete theform. You must sign the declaration on the medical report form in the presenceof the registered medical practitioner. It must be submitted within 1 month ofthe medical examination.
Eyesight report form
Generally any eyesight issues will be reported in the Driving LicenceMedical Report Form. But, you will need a Driving Licence Eyesight Report Formif you no longer need glasses or contact lenses (for example if you've hadlaser eye surgery). You can downloadthe eyesight report form (pdf) (pages 1 and 2 should be printed back toback on one page). It is also available from NDLS centres. It must be submittedwithin 1 month of completion.

Stating your driver’s license on your resume should be pretty straightforward. You should state what type of certified Driver’s License you have. As different classes of licenses are necessary to drive various categories of vehicles. For those applying for a driving job, your safety record and years of driving experience are required. They should appear earlier since such information significantly influence hiring decisions for such jobs. Ensure you are familiar with the categories of licenses and what kind of license makes you eligible to drive what kind of car. This puts you on the same page with your prospective employer and you can be sure you are eligible for the job you are applying.
Renewing Irish Driving Licence
What not to state
You should be careful about adding extra information to your resume as a voluminous resume could put off hiring managers. Be caution about including sensitive information. Your driver’s license number is one such sensitive piece of information. Your identity could easily be stolen if such information is revealed and you never know where your resume is going. Other sensitive information you should leave out of your resume. For security reasons never include your social security number or credit card information.
